School-Based Health Program
DeRuyter Central School offers a unique program called the DeRuyter School-Based Health Program (SBHP), sponsored by the Family Health Network. The clinic is an all-inclusive doctor’s office conveniently located in the school, and is available with no out of pocket costs. Services offered by the clinic include: treatment of acute illness, monitoring of chronic illness, physicals, immunizations, allergy injections, behavioral health screening and medication management, and other general healthcare needs. All students enrolled in DeRuyter Central School are eligible to enroll in the School-Based Health Program.
If you wish to have your child seen for an acute illness, remember you may notify the DeRuyter School-Based Health Program by note or phone call. If your child is home ill, you may also call for an appointment and bring your child in to be seen. The School-Based Health Program is open when school is in session. If your child needs to be seen during a school break or holiday, you can contact any of the Family Health Network clinics for an appointment (familyhealthnetwork.org).
In addition to our medical services, our School-Based Health Program also provides annual preventative dental services to K, 2nd, 3rd, and 6th graders. The dental services include oral health education and promotion, dental screenings and referrals, sealants, cleanings, fluoride varnish applications, case management and assisting in the establishment of a dental home as needed. Be on the lookout for a green folder to be sent home with your child. Please sign and return all forms.
If you have any questions about the School-Based Health Program or would like to enroll, please contact Val at 315-852-3400 x8.
